Thorough reporting and analysis of key metrics and outcomes improve not only how organizations identify and respond to adverse events but also how they proactively identify potential risks. It also helps underscore “the why” behind why initiatives have been put in place. The journey toward high reliability in healthcare gained momentum more than two decades ago with the release of the Institute of Medicine’s (IOM) landmark report, To Err is Human (1999), which revealed shocking statistics about preventable deaths in hospitals. This pivotal report was a turning point and catalyst for healthcare organizations to move beyond complacency and take definitive action to reduce harm and improve patient outcomes.
